IMPORTANT NOTES BEFORE YOU COMPLETE REGISTRATION!

INFORMATION WE COLLECT DURING THE REGISTRATION PROCESS

• If purchasing the optional printed syllabus (see below), please note it will not ship a few weeks before the course. We will ship to the address you provide at the time of registration. If your address changes between the time you register and when syllabi are shipped, you must inform us of the new address to ensure the syllabus gets to you. Additional shipping charges may apply.
• We ask for your ABIM ID# to facilitate the CME credits earned for taking the course. If you don’t know your ABIM ID#, CLICK HERE to retrieve it (this will open the ABIM website in a new tab so you don’t interrupt your registration).
• Please provide an active email address. All communication regarding your account and access to the Steinberg Course materials will be via email to the address you provide.

OPTIONAL ADD ON MATERIALS

• The printed syllabus is NOT included with registration! Your registration includes only the downloadable syllabus. If you wish to receive a printed syllabus, it must be purchased separately!
• Registrations submitted without a valid Physician, Physician Assistant, Nurse Practitioner, or Fellows ticket will be cancelled and refunded. A processing fee applies to all refunds.

Notes on email addresses

We ask for personal email addresses for registrants because your subscription may still be valid if your fellowship ends and/or you change employers. Having your subscription attached to your personal email address ensures no interruptions in your studies. 

Conversely, for Fellows, we ask that the email address for your director be an institutional email address for verification purposes.
